Attention students who received shot forms:

Please take home these permission slips and return them to the nurse by Friday, February 22nd, if your parents want you to get the shots at our Health Department Clinic at school. We expect many students to participate, so they do not have to do this over the summer vacation. You should turn in your payment with the form, except for students who have medicaid--they should have their parents write their number on the form instead of sending payment. If you have questions, please see the nurse.
